ACTIVEHIP+ es un programa educativo y de tele-rehabilitación para pacientes que han sido intervenidos quirúrgicamente tras sufrir una fractura de cadera o han necesitado una prótesis de cadera, que incluye además una sección específica de formación y asesoramiento para sus cuidadores. El objetivo del programa es mejorar la recuperación funcional de las personas que han sido intervenidas quirúrgicamente, favoreciendo su independencia para realizar las actividades de la vida diaria y contribuyendo a mejorar su calidad de vida. Al mismo tiempo, es una herramienta de asesoramiento y formación para los cuidadores, que busca disminuir las lesiones en cuidadores y mejorar su calidad de vida de los cuidadores. ACTIVEHIP+ es además un programa diseñado para prevenir caídas y fracturas secundarias. El programa de telerehabilitación ofrece dos sesiones semanales de ejercicio físico y una de terapia ocupacional, graduadas en dificultad y que son supervisadas por profesionales sanitarios, los cuales las adaptan y modifican, individualizando los tratamientos según las necesidades de cada persona. Los contenidos educativos, incluyen descripciones sobre el tipo de operación, cuidados de la herida quirúrgica, recomendaciones previas al alta y al llegar a casa, recomendaciones dietéticas, dispositivos de apoyo a utilizar durante las primeras semanas para realizar las actividades de la vida diaria, y recomendaciones para prevenir caídas y prevenir fracturas secundarias como adaptaciones del hogar, fármacos y actividad física. El programa ha sido diseñado por terapeutas ocupacionales, enfermeros, médicos de familia, fisioterapeutas, médicos traumatólogos, médicos rehabilitadores, nutricionistas, y licenciados en ciencias de la actividad física y el deporte. ACTIVEHIP+ incluye además un sistema de videollamadas y de mensajería entre usuarios y profesionales sanitarios.
