Every question goes through a separate review pass over the answer, the wrong options and the explanation, and automated checks run across the whole bank; anything the review cannot settle is held back rather than published. It is a careful process, not a perfect one - if you find a question that is wrong, report it straight from the question in any mode, or write to [email protected], and it gets fixed in the next update.
