Real-time appointment scheduling, view lab results, check vitals, message your provider, manage medications, mobile video visits, complete forms and more from one place. With My Premise Health you can: • View your health record • Book and manage appointments • Message your providers and care team • View lab and test results • View current medications and refill prescriptions • Conduct virtual visits • Complete forms and eCheck-In • View billing history, statements and make payment online • View your immunization history and health reminders • Manage your dependent’s health information • Upload health and fitness data, including data from Health Connect, when enrolled in self-tracking programs. It’s never been easier to get, stay and be well. To use the app and access your health information, you must first create an account through your healthcare provider or have an active My Premise Health account. Don’t have an account yet?
