Notermed: AI Medical Scribe

Notermed is a medical scribe for clinicians. You record the consultation on your phone, and a structured clinical note comes back drafted from the conversation. You need a Notermed account Sign in with the email your clinic administrator set up for you. If your practice is new to Notermed, tap "Create an account" on the sign-in screen: sign-up happens on notermed.com and takes a few minutes. The app does nothing useful without an account. What the phone does • Records the visit, with speaker separation between you and your patient • Keeps every recording on the device until the server confirms it arrived, so a dropped signal or a closed app does not lose a visit • Lets you record first and attach the patient afterwards, for visits that start before the chart is open • Shows your visit list and the drafted note • Secure messaging with your clinic team, with photo and file attachments • Tasks you can assign, comment on and close, so follow-ups do not live on sticky notes • Notifications for new messages and tasks that never name a patient on the lock screen • Unlocks with fingerprint or face recognition What stays on the desktop Editing and signing happen on the web app. A signature is a clinical attestation, and a phone in a corridor is the wrong place to make one. Editing clears any signature on the server, so it is deliberate work as well. The notes SOAP, H&P and follow-up formats, drafted in your own templates and your own phrasing. Every note is a draft until you sign it. Until then it stays outside the record. Privacy Recordings, transcripts and notes are encrypted from the exam room through to storage. Your data is stored and processed on servers in the United States only. We sign a Business Associate Agreement with your clinic before any patient record is processed. Your patients' conversations are never used to train AI. Notermed drafts documentation from what was said during the visit. It does not diagnose and it does not make clinical decisions. You review and sign every note.
