O projeto UHD corre sobre uma plataforma que permite a interação remota com os pacientes, a monitorização regular do seu estado de saúde, a priorização de intervenções consoante a evolução da situação clínica e a intervenção no domicílio dos doentes sempre que necessário. A oferta é estruturada em “clinical pathways” que definem a abordagem otimizada às situações clínicas que vamos servir. Esses “pathways” são depois traduzidos em planos individuais de cuidados que refletem a necessidade pessoal e única de cada paciente. Esses planos multidisciplinares permitem a execução adequada e atempada dos atos e o registo da informação clínica relevante e geram uma alarmística para profissionais e pacientes para atuação no momento certo e da forma certa. Esta aproximação da situação clínica dos doentes permite uma visão global sobre a sua situação e a manutenção de um historial clínico que suporta decisões clínicas fundamentadas e atempadas. Tudo isto em casa, sem deslocações desnecessárias.
