Moon Valley Med Spa is a medical spa in north Phoenix. This is our patient app. It is where you book your visits, keep track of them, and get the paperwork out of the way before you arrive. You are looking at our real schedule, so anything you can see, you can book. WHAT YOU CAN BOOK Injectables and medical: Botox injections, medical weight loss, B12 shots. YOUR VISITS IN ONE PLACE Upcoming and past visits, each with the details. Something come up? Reschedule or cancel from your phone, without having to call us during business hours. FORMS BEFORE YOU ARRIVE Fill in your intake and consent forms at home, on your own time, instead of on a clipboard in the waiting room. We tell you when there is something to sign, and you sign it with your finger. REMINDERS, IF YOU WANT THEM Turn on notifications and we will remind you about an upcoming visit. You choose what you hear from us about, and you can change that whenever you like. MEMBERSHIP AND CREDITS If you are a member, your plan and your remaining credits are in the app. SIGNING IN There is no password to remember. Enter your phone number or your email address and we send you a six digit code. YOUR INFORMATION, AND HOW TO REMOVE IT You can close your account from inside the app at any time. Go to Profile, then Delete my account. We will show you anything still outstanding first, such as a booked visit, so nothing catches you out. We are a licensed medical practice, so some treatment records have to be kept after your account closes. Arizona law sets those periods, not us. We spell out exactly what is deleted and what is kept, and why, at moonvalleymedspa.com/data-deletion This app is for patients of Moon Valley Med Spa. It does not give medical advice and it is not a substitute for talking to your provider. If you are having a medical emergency, call 911.
