The Optum Homecare app helps you stay connected to your care team from the comfort of home, making participation in your care program simple and convenient. If you are experiencing a medical emergency, call 911 or seek immediate medical attention. Availability: This app is offered in the United States only. Free to download for eligible Optum Homecare patients. ------------------------------------------------------------ Regulatory Information: Requires the compatible Bluetooth blood pressure monitor provided through your Optum Homecare program. The app aggregates and displays health readings collected by that device and, where you grant permission, from Android Health Connect. It does not measure these values, and it does not alter, validate, or interpret the readings it receives. Health data handled by this app: • Blood pressure (systolic and diastolic) • Heart rate (the pulse recorded with each blood pressure measurement) Health Connect: This app requests only the Health Connect permissions it uses — blood pressure and heart rate. It does not request steps, activity, sleep, weight, blood glucose, oxygen saturation, body temperature, or any other data type. Heart rate is requested because the Optum Homecare blood pressure monitor reports a pulse rate with every measurement. The app reads heart rate to show that pulse alongside each reading in one consolidated history, and writes it back so your own health record stays complete. This app is not a medical device and does not diagnose, treat, cure, or prevent any medical condition. It has not been independently evaluated or cleared as a medical device. USERS MUST CONSULT WITH A HEALTHCARE PROFESSIONAL FOR MEDICAL ADVICE, DIAGNOSIS, OR TREATMENT, AND BEFORE MAKING ANY MEDICAL DETERMINATIONS.
