With EmmaCare you will be able to reduce your gaps in care and provide your Care Manager with essential and real-time information about your health. Your interactions with your Care Manager will become much more meaningful and will focus on your specific healthcare problems. With EmmaCare your Care Manager will: Help you manage your specific illness: - Report vital signs via our connected medical devices - Medication reminders - Periodically check-in on your health - Schedule your weekly/monthly appointments - Provide you with logistical help All of the above and the fun to earn rewards!!!
