Registering for the My Circle Health App - To use the My Circle Health App, you must already have an appointment scheduled at a Circle Health Group hospital. - Registration also requires your 8-digit EMPI (patient number), which can be found on your My Circle Health email or SMS invitation or any appointment letters. - You’ll also need to enter the mobile number, email address, date of birth, and surname exactly as recorded in your patient record. If you don't have an appointment yet, you can visit our website to book a new appointment online: https://www.circlehealthgroup.co.uk/
