CardiRehab: Home Cardiac Rehab

CardiRehab brings cardiac rehabilitation to your home. If you are recovering from a heart attack, angioplasty or stent, or living with heart failure, you can follow the programme your cardiac rehabilitation team sets for you and stay in touch with them from your phone, without travelling to a hospital for every session. WHAT YOU CAN DO • Follow your weekly plan See the activities your specialist has set for each day and mark them done as you go. • Keep an exercise diary Log how long you exercised, your peak heart rate and how hard it felt on the RPE (Borg) scale. Your specialist can see every entry. • Complete assessment forms Fill in the PHQ-9, GAD-7, HADS, six-minute walk test, and alcohol and tobacco questionnaires your care team asks for. • Message your care team Send text, voice messages, photos and files, and get a notification when they reply. • Ask for a second opinion Send your reports and echo or angiography videos with your question, and read the answer in the app. • Learn about heart health Guides on cardiac rehabilitation, living with heart disease, reducing your risk, diet, alcohol and smoking, with exercise instructions and an RPE scale you can download. • Find trusted resources Hospitals, government schemes, and diet, exercise and learning resources for your heart-health journey. FOR CARDIAC REHABILITATION SPECIALISTS See the patients assigned to you, message them, and review and answer their second-opinion requests. AVAILABLE IN SIX LANGUAGES English, Hindi, Telugu, Tamil, Kannada and Malayalam. YOUR DATA Everything you send travels over an encrypted connection. You can delete your account and your data at any time, in the app (More → Edit Profile → Delete Account) or at www.cardirehab.com/delete-account.
