香港路德會社會服務處復康組於2021年開展「路同行」殘疾人士家庭支援計劃，旨在接觸殘疾人士照顧者，辨識和了解他們的需要、及早介入和處理他們的情緒壓力問題。期望透過本應用程式，提供平台讓殘疾人士照顧者了解自己的精神健康，並汲取有用資訊和活動資料，從而得到支持；除此，廣大市民亦可了解自己的精神健康，和認識本計劃，並參與支援殘疾人士家庭。此應用程式提供以下功能： - 照顧者互動平台 - 壓力情緒測試 - 有用資訊 - 活動花絮 - 聯絡職員
