高知家＠ライン ヘルスケアは、高知大学が開発し一般社団法人高知医療介護連携システムが提供する、健康づくりをサポートするアプリです。 ご利用は無料ですが、ご利用時の端末費用および通信料（パケット費用含む）はご利用者の負担となります。 現在、以下のことができます。今後、順次機能拡充を予定しています。 ・ポイント獲得機能：ウォーキングや血圧測定などの健康づくりに取り組むことでポイントが貯まり、取り組みの成果を確認できます。 ・各種健康管理機能：服薬状況やバイタル情報、食事情報を記録・管理することができます。 ・各種チェック機能：フレイルチェック、基本チェックリスト、後期高齢者の質問票で心身の健康状態を把握きます。 ・診療情報確認機能：医療機関で受けた検査結果や処方情報（薬剤名・用法・処方日など）を確認できます。 ・ACP参照機能：万が一のときに備えて、どのような医療やケアを望んでいるか、本人による意思決定を参照できます。 これらについて、自分だけでなく家族の情報も登録・管理することができます。
