Шкала оценки органной дисфункции SOFA была предложена для оценки мультиорганной недостаточности у пациентов с сепсисом. Изначально она называлась шкалой оценки тяжести органной дисфункции при сепсисе (The Sepsis-related Organ Failure Assessment, SOFA Score), но в дальнейшем ее название было изменено на современное - Sequential Organ Failure Assessment (динамическая оценка органной недостаточности). Изменение названия было обусловлено пониманием факта, что мультиорганная дисфункция может быть вызвана не только сепсисом, но и другими патологическими процессами различного происхождения. Индекс SOFA равен сумме шести показателей. Чем выше показатель, тем больше недостаточность оцениваемой системы. Чем выше индекс в целом, тем больше степень мультиорганной дисфункции. Нарушение функции каждого органа (системы) оценивается отдельно в динамике на фоне интенсивной терапии. При сумме баллов не более 12 предполагаются множественные органные дисфункции, 13—17 баллов — предполагается переход дисфункции в недостаточность, сумма баллов около 24 указывает на высокую вероятность летального исхода. При наличии инфекции увеличение индекса SOFA на 2 и более баллов по сравнению с предыдущим значением является одним из клинических критериев сепсиса.
